What is an FDA close-out letter?

A close-out letter is FDA's notice, issued after it has completed an evaluation of the corrective actions a firm took in response to a warning letter. FDA says it does not issue one on promises that action will be taken. The corrective actions must have been made and FDA must have verified them.

What FDA says

FDA's page on warning and close-out letters says the agency may issue a close-out letter once it has completed an evaluation of the corrective actions taken in response to a warning letter and finds the firm addressed the cited violations. It says FDA will not issue one based on a firm's representations that it will take action. The corrections must have been made and verified.

Where to find one

FDA's warning-letters table has a close-out letter column. An empty column means none is posted. It does not prove the firm never responded, and a missing close-out letter does not by itself show the violations continue.

How to read the status

  • No close-out letter posted: the letter's issues may be unresolved, or FDA may not have evaluated the response yet. We cannot tell which from the table alone.
  • A close-out letter posted: FDA says it evaluated the corrective actions for the violations cited. Read FDA's page for what that does and does not cover. Do not read it as approval of the firm or its products.
  • Our index does not track close-outs, so check FDA's page for each letter.
